The Ontario Early Years Centre of SD&G is proud to offer a safe and healthy environment. While at the Centre, I understand that I am ultimately responsible for the safety and well-being of the children under my supervision. I also understand that the Ontario Early Years Centre of SD&G, its staff, volunteers or students are not liable in the event of injury, sickness or death occurring during my attendance at the Centre. I have read and understand the policies and procedures of the Centre.
The information collected by the Ontario Early Years Centre of SD&G is for the sole purpose of administering the Centre. If further details are required about the collection of information, contact a representative of the Centre.
Terms of Use
I will do my best to take care of the toys/equipment borrowed and will return them on the due date. I have read and agree to follow the toy/equipment library policies of The Ontario Early years Centre to SD&G. I understand it is up to the parent/caregiver to choose toys/equipment that is age appropriate and to supervise their use. I understand that The Ontario Early Years Centre of SD&G toy/equipment library, its staff, the board, and/or volunteers are not liable in the event of injury, loss or death sustained from the toys/equipment borrowed from our library.
